The South Manchester Community Falls Team will be visiting and delivering a presentation on falls prevention. There will also be opportunity for blood pressure checks. Please speak to Lucy F for more details.
The Community Falls Team is a specialist multi-disciplinary team consisting of a Physiotherapist, Occupational
Therapist, Falls Nurse and Therapy Assistant and also includes sessional input from Podiatry.
The team provide a home assessment for patients who have fallen or who are at high risk of falling and who have the
potential to engage in a falls prevention programme.
The aim of the service is to provide an individual treatment plan to highlight potential falls risks, maximise
independence, restore confidence and reduce the risk of future falls.
o Environmental checks and equipment provision to reduce the risk of falls
o Tailored exercise programmes to improve strength and balance
o Walking aids to reduce risks of falls
o Osteoporosis/ fracture risk assessment
o Lying and standing blood pressure checks
o Medication advice
o Falls management strategies
o Strategies to help with fear of falling and confidence building
o Signposting to community activities, groups and exercise programmes
o Aged 60 and over
o South Manchester GP
o Not living in a nursing or residential home
o Mobilising independently (with or without aids)
o Recent history of one or more falls or at high risk of falls/fractures
o Cognitively and physically able to engage in a falls prevention programme
o Not currently known to other community therapy services
o Medically stable
